Kids Academy Welcome Letter, Curriculum Schedule, Parent Handbook and Enrollment Packet

As a prospective Parent or Guardian, we want to welcome you. In this letter you will find many things, including our DHS License Number along with our Contract Number which is needed for Subsidy activation.

This is the Child Enrollment Packet. Each child applying must have all paperwork filled out and turned in prior to entering the program.

The packet contains the following:

Child Information Form-Must be filled out entirely, signed and dated

Child Developmental History Form-Must be filled out to the best of your knowledge

Authorization for Emergency Care to Minors-This form is basically giving the Kids Academy at SHUMC the authorization to seek medical care for your child in the event of an emergency.

Health Status Authorization and Photo Release-Fill it out and provide what is required for entry.

Compliance File Notification-This is a form that says you are aware that we are Licensed by OKDHS Childcare Licensing, and you are aware that we have a Compliance File On-site and Online by going to OKDHS.ORG. Ours is located on the Welcome Desk.

Immunization Record Authorization Form-Please fill this out so I can pull the shot record for your child.

Exemption- If your child has a MEDICAL Exemption, we will just need a Doctor Statement to keep on file. If you have a PERSONAL OBJECTION or RELIGIOUS EXEMPTION, you must follow the instructions on the form related to obtaining an Exemption. The exemption must be approved to be enrolled.

Sunscreen and Bug Spray Authorization-Fill out

Parent Handbook Acknowledgement-This form is stating that you have acknowledged that you have read and agree to the Parent Handbook that is posted above.

Building for the Future-A letter regarding the CACFP food program that Kids Academy Participates in.

Letter to the household-A letter regarding the reason for the FSIA participant application.

Instructions on how to complete the FSIA.

CACFP FSIA and Enrollment Form-Please fill out as instructed.

Our Curriculum is completely Individualized for EACH child in our program. We want to work with you to set goals for your child. In addition, if your child is working with SOONER START and has an IFSP or with Moore or OKC Public Schools and has an IEP we would love to have a copy so we can continue to work on those goals here at the Kids Academy.

Along with our individualized curriculum, our center partners with GO NAPSACC, Go NAPSACC works with childcare providers to improve the health of young children through practices, policies, and environments that instill habits supporting lifelong health and well-being. It covers many areas and key topics like healthy eating, physical activity, and oral health.

We also received the Certified Healthy Certificate of Excellence for Early Childhood Programs in 2021, 2022, 2023, 2024 and 2025.
